Quality Assurance Inititatives
Passageway is engaged in Continuous Quality Improvement to ensure a high standard of services to our persons served. Some of our main initiatives include the following:
Case Management Services
Liaison, advocacy, and personal management services designed to assist the individual manage daily living tasks, obtain resources needed to successfully cope with mental illness, and link both the individual and their families to supplemental resources available in the community.
Community Integration (COI)
Passageway provides groups to residential and outreach person served on topics such as budgets, healthy cooking, housing, shopping, employment readiness skills, mental health and symptom management, substance use and recovery, social support system, community resources, and self-care to name a few. Individual and Group Therapy Services
From the initial contact during the admission process, Passageway staff administers trauma assessments and screenings. When an individual requires trauma treatment, he or she will be assigned to Passageway’s onsite therapist.
Medical and Psychiatric Consultation Services
Passageway is committed to providing an integrated system of care that incorporates comprehensive screening and monitoring of those individuals affected by chronic health conditions and a system of care that meets their needs. Residential Services
Passageway residential treatment program is designed to integrate individuals with behavioral health or co-occurring needs back into the community and living independently whenever possible. Passageway is staffed 24 hours a day, 7 days a week by trained personnel who provide safe, trauma-informed, recovery-focused services and environment.
Independent Living (Outreach)
Upon completion of our residential program, Passageway persons served can transition to independent living in one of our secure, supervised housing programs. Persons served will continue to receive medication and case management services in addition to in-home assistance in enhancing skills related to performing household tasks.
